FEES FOR SERVICES
Our office is a “Fee for Service” office, which means that payment for treatment is required the day the service is provided.
We kindly accept payments by Visa, MasterCard,Debit and Cash.
We do not accept assignment from insurance companies.
INSURANCE CLAIMS
Our office is also equipped to process your insurance claims by Electronic Data Information and we look forward to looking after your insurance needs. We do remind you that you are responsible for your account and the insurance company is responsible to you.
We do not have any information regarding your personal policy and cannot access any details of your policy due to the privacy legislation of Ontario.
We will be happy to provide an estimate for any dental treatment based on our professional assessments. This may be forwarded to your insurance company as dental treatment needed may exceed your dental benefits.
APPOINTMENT CHANGES
We kindly request that you give us one (1) business day notice prior to a change in your appointment status or a $50.00 charge will be applied to your account.
Our answering machine does not accept messages for changes in appointments.
Please call the office on the next business day to make alternate arrangements.
